The book is accepted and contract signed with agent, nothing is heard for several months and though repeated requests for update regarding progress with book nothing happens, is this usual?

Dear Ann

It isn't unusual for it to take some months for an agent to sell your book to a publisher, but you should be kept in the loop. Every Agent-Writer relationship differs, but you should be in contact at least once a month. It is perfectly acceptable for you to ask your agent which editors they have submitted your book to and ask to see the decline letters.
