Did you work to a template with ready-made characters and working chapter titles, or did you write without a plan?

I never write with out planning my book out(the basic plot that is) I've got an assortment of character's ready before I begin, but if i have an idea for a good one, I add he/she in. I also make-up the chapter titles as I go along.

I started with the beginning, middle and the end. the rest was added on the way.
