I can't remember where I read it (rather unhelpfully), but I saw a really effective short story about a couple at a dinner party. They were asked how they met and the first half of the piece goes into loads of detail about the past and why the fact that this character smoked was absolutely central to her having met her husband. Then at the end it is revealed that she has cancer, but hasn't told him. Doesn't sound particularly effective broken down like that, but it was definitely one of those 'gut-punching' endings at the time.
The ending of Artemis Fowl for sure. :)
Probably the best one I've read, but Harry Potter does have a great one and comes quite close.

Sorry. Didn't mean to compromise what anyone was writing.
I Would like to change my question to: --- "What is the best ending or twist you've Read!"
