Admin and David, the problem lay with me and a new computer with Win 8.1 - it's a new thing for me that in spam, no links are live; how to confuse a non-techie writer in one easy stage.
Moved it to inbox and it worked. Thanks for your help.

I've just opened the email and the link to the questionnaire seems to be ok now. Just filled out the questionnaire and it submitted no problem.
