I try to write stuff in sequence, beginning to end, mainly so small plot points that might be relevant later don't get lost or forgotten. But in my first story, the beginning ended up in the middle, and in the second the last chapter ended up as the prologue. So it don't always work out as planned :?
Before starting to write, I just scribble out a thousand words and then it is just expressing yourself. I never make much pre preparations, I kinda love surprises and exciting occurrences!
I started with key scenes - major plot developments, defining character moments, etc - and filled out the bits in between. Not how I would necessarily recommend going about it, but it was useful in determining the shape of the book. I then of course left myself with the nightmarish task of writing an opening that fit the rest of the book. The other problem, that of sagging in the middle, kind of solves itself, as you automatically tighten the pace on the 'bridging' chapters. It only works if you have a good outline beforehand...
