How do you keep track of your chapters? Do you write each chapter in separate files or the entire book in one large file?

I have a chapter breakdown page ... a brief synopsis of each under the chapter headings.

I use one large file. Before chapters are written there tends to be a few words at the top of each in capitals stating what I expect to appear in that chapter. This can range from a single word to a detailed explanation which I only erase that when the chapter is complete.

I also keep a record of chapters with their current page numbers on the wall in front of me for quick reference.
