How do you keep track of your chapters? Do you write each chapter in separate files or the entire book in one large file?

Yeah write all in one doc, though if editing a certain section may lift it out into a fresh doc (with other relevant/timeline sections if necessary).
I already have a plotted chapter plan, but also write a chapter bios once written as the two may often differ. (Hand written)
I also have the plot points on post its, in order of timeline, above my desk so I have a good visual of ideas in play, or how changes in structure would play out.
And then in my second book I have started to write an almost stage direction style chapter plan for the next section of plot ahead of what I'm writing. I find this really helpful with movements of secondary characters and expressions of motif/theme.

I used to write all the chapters in one file : bad idea, spaghetti dish !

All my chapters from the beginning to end of my novel, are on a single Microsoft Word document.
I have separate Word documents with the Section and Chapter Titles with page numbers, Synopsis, Covering Letter, Cover Design and Blurb.
